You will need to login to the ADP Benefit Enrollment Module to enroll or review your benefits and complete the Pre-tax election form (forms must be printed, completed, and uploaded back into ADP before you submit your enrollment. If you need help with this process, please see your HR Coordinator).

Remember, annual re-enrollment is required for the Flexible Spending Account (FSA) and the Health Savings Account (HSA) plans each year. If you do not re-enroll in the FSA or the HSA your current elections will not rollover.

The wellness program is with PHMP, and participation will be for team members only. Team members will have to enroll in the PHMP wellness program to be eligible for the discounted medical rates. If the team member would not participate for 2 consecutive months in the wellness program, then the team member will lose the discounted medical pricing and be moved to the higher premium until the next benefit renewal. Monthly involvement via text, email or phone is required.

For all Full-Time team members currently enrolled in the PHMP Wellness program the Biometric testing forms are available online through December 31, 2024. We want to make sure that each team member enrolled in the wellness plan has the option to participate in the biometric screening because this is a FREE amazing benefit that can improve team member health and help decrease health plan cost at the same time. We have two options for team members to complete the testing.
  • Lab Corp testing facilities.
  • Request a Home Test Kit – mailed directly to home postage paid by PHMP. See QR Code for direct link.
Remember, this program is for Buchheit team members currently enrolled in the wellness program and Spouse will not be eligible for the free testing. There is no cost to participate in the biometric testing.

The directions will show how to schedule appointments with Lab Corp, as well as requesting a home test kit to be mailed to your home address. If scheduling at a Lab Corp facility, make sure to print the voucher to take with you.
  • Remember, to get accurate test results it’s best to fast for 8 hours prior to the test. No food and only drinking water or black coffee.
Remember, the biometric testing should be completed by the end of December and will be a requirement to keep the medical discount premium. Any team members that opt out of participation or do not complete the biometric test by the end of December will pay a higher medical premium.

You agreed in your application on the Acknowledgement of Terms of Employment or Contract/Lease Agreement to have or to obtain a Hazardous Materials Endorsement (HME) within 90-days of joining the Buchheit Team.

ELDT is required for a first time Hazardous Materials Endorsement (HME) applicant. A hazardous materials endorsement requires completion of a knowledge test. The ELDT must be completed prior to appearing a State Highway Patrol Driver Examination Station to be eligible for testing.

The ELDT regulations defined by the Federal Motor Carrier Safety Administration in 49 CFR 380 establish minimum training requirements for entry-level commercial motor vehicle (CMV) operators in interstate and intrastate commerce who are applying for:

  • A Class A or Class B commercial driver’s license (CDL) for the first time;
  • A Class C CDL with Passenger, School Bus or hazardous materials endorsement for the first time;
  • An upgrade of their CDL (e.g., a Class B CDL holder seeking a Class A CDL); or
  • A hazardous materials (H), passenger (P), or school bus (S) endorsement for the first time.

The requirements do not apply to individuals holding a valid CDL or an H, P, or S endorsement issued prior to February 7, 2022. 

Steps for obtaining a HME

  1. If you have previously had a HME, skip to step #2. If obtaining the HME for the 1st time ever, complete the ELDT issued in orientation via Driver Pulse: https://pulse.tenstreet.com
  2. Pre-enroll for your criminal background check via TSA and Homeland Security: https://www.tsa.gov/for-industry/hazmat-endorsement
  3. Visit an application center to complete your enrollment and get fingerprinting done:
    1. Provide required documentation and fingerprints. Bring your current U.S. passport or a driver’s license and birth certificate. See other acceptable documents.
    2. Pay a non-refundable fee valid for five years with a credit card, money order, company check or certified/cashier’s check.
      1. New and renewing applicant: $85.25
      2. New and renewing applicant reduced rate with TWIC: $41.00*
    3. You can check your status online at any time.
  4. After your background check is complete go to your state’s DMV to take the test for the HME and add it to your CDL.

Hazardous Materials Transportation Training from PHMSA

The Hazardous Materials Transportation Training Modules introduce users to the Hazardous Materials Regulations (HMR; Title 49 CFR Parts 171-180). All hazmat employees (as defined in § 171.8 of the HMR) must receive training that includes general awareness/familiarization training, function-specific training, safety training, and security awareness training. See Subpart H of Part 172 of the HMR for all training requirements.

The Office of Hazardous Materials Safety (OHMS) has developed this free online training module program to meet the requirements for general awareness/familiarization training and to serve as the basis for function-specific training. The “8.0 Security Requirements” training module meets the requirement for security awareness training.
